You can download a form here - or you can ask for one when sending your cheque and one will be posted to you.  Please do not send this form direct to your bank - please send it to the office at the address printed on the form. You are invited to increase the value of your gift (an additional 25p for every pound donated), if you are a U.K. taxpayer, by making a Gift Aid declaration.  A Gift Aid declaration can be made on the Standing Order form
